This form is to allow students to notify the Academic Dean’s Office when they cannot attend class due to illness or hardship. Please note, when illness exceeds two days in duration, you will be asked to provide a doctor’s verification of your illness. Please consult the Office of Academic Affairs or the Office of Student and Alumni Affairs if you have questions.

When illness or hardship is not involved with an absence it is the class instructor that decides if a student may be excused from a class. For example, if you wish to miss class for a conference/student meeting out of town, etc., you should FIRST speak to the instructor to obtain permission to miss class. After you have approached your instructors and have their approval for an excused absence THEN notify the Academic Dean’s Office about the pending absence through the this link. Simply filling out the Student Absence Notification Form is NOT sufficient when seeking an excused absence.
